What we collect

When you submit a "request a visit" form we collect your name, the phone number and/or email address you provide, your preferred visit time, and any message you write. We ask you not to include health information, and we do not request it.

How we use it

Your request is forwarded to the office and agent you selected so they can contact you about scheduling a visit. We do not sell your information.

Cookies and analytics

This site does not use advertising cookies. We may use privacy-respecting, aggregate analytics to understand which pages are useful. Office pages embed a map from Google, which may set its own cookies when the map loads.
